Did you take your child for immunization against measles on any of the suggested date?

 Background
Measles is the most deadly of all childhood rash/fever illnesses. The disease spreads very easily, so it is important to protect against infection. To prevent measles, children (and some adults) should be vaccinated with the measles,) vaccine. Three months ago there was an outbreak of Measles in the districts of western Uganda and the government of Uganda declared 3 days, from (3rd- 5th October), for all the Children from 6 months- 5 years to be immunized against Measles.

A.      Yes I took all my children of that age bracket  for my Immunization
B.      No I did not take  any of  my child  for immunization even when I knew about it
C.      No I never took my  children for immunization because I was not informed
